About this policy
This policy explains how The Honest Collective (ABN 57 682 770 994) collects, uses, stores and shares personal information. We follow the Privacy Act 1988 (Cth) and the Australian Privacy Principles and, for health information, the Health Records Act 2001 (Vic). It applies to participants, families, carers, referrers and anyone who contacts us.
What we collect
Information you or someone acting for you gives us, such as your name, contact details, date of birth, address, NDIS number and plan details, your goals and support needs, and health or disability information relevant to your support. When someone sends us a referral, we also collect their name, their relationship to you and their contact details. You can make a general enquiry without giving your name, but we need your details to provide supports.
How we collect it
Directly from you, or from someone acting for you such as a family member, carer, support coordinator or plan manager. This may be through our website, by phone, by email or in person.
Why we collect it
To plan and provide your support, manage your service agreement, claim payment, meet our obligations as an NDIS provider, and keep in touch with you.
How we keep it safe
We keep records in secure, password-protected systems, and only team members who need your information to support you can access it. Our team is trained in privacy and confidentiality. We keep records for as long as the law requires, which is generally at least seven years after your last support with us, and then securely destroy or de-identify them.
Seeing or correcting your information
You can ask to see or correct the information we hold about you at any time. Call us on 0452 140 662 or email hello@thehonestco.com.au.
Making a privacy complaint
Contact us first and we'll respond within 30 days. If you're not happy with our response, you can contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or, for health information, the Health Complaints Commissioner (Victoria) at hcc.vic.gov.au.
